Very thorough Ajarae! Great tips. Another thing would be to look them up on IMDb, see if any friends or acquaintances have worked for them and ask them to introduce you, maybe set up a general meeting or you can put together a package/letter for them or email with note, photo, reel and resume for the friend to pass on to the CD.
